3. Burial/Transit Permit
4. Embalmer's Affidavit
5. Letter of non-contagious disease
6. Funeral home letter stating: 1) That only the embalmed remains are in the casket, 2) flight itinerary; 3) consignee's name, address, and telephone number; 3) cemetery name and location
7. Casket - Metal, sealer-type
8. Zinc-lined wood shipping box
