United Kingdom - Shipping Requirements

Requirements:

Transport of Human Remains

1. Certified copy of Death certificate

2. Burial/Transit Permit

3. Passport of decedent

4. Embalmer's affidavit along with statement of contents of casket on funeral home letterhead

5. Letter of non-contagious disease from attending physician

6. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;

7. Sealer casket with sawdust or charcoal for absorbtion

8. New wood bottom base Air Tray

9. Body must be in plastic pouch

10. Zinc liner required if wood casket is selected

11. No Consul legalization, fee or inspection.

12. Documents should be shipped with remains

Transport of Cremated Remains

1. Certified copy of Death certificate

2. Cremation Certificate

3. Statement on Funeral Home letterhead that the urn contains only human remains
