Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport, birth certificate or marriage certificate of decedent
4. Embalmer's affidavit (notarized with a certificate of notary)
5. Letter of non-contagious disease from State or County Health office
6. Letter of preparation on funeral home letterhead, notarized and authenticated
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
8. No requirements on casket
9. Zinc lined hermetically sealed wood box
10. No consul inspection, legalization fee is $52.00 per each document
11. Family must go to the Consulate office with the above documents
12. Passport and ID of person submitting papers

Transport of Cremated Remains

1. Certified copy of Death certificate
2. Cremation Certificate
3. Flight Itinerary on funeral home letterhead
4. Passport, birth certificate or marriage certificate of decedent
5. Family must go to the Consulate office with the above documents
6. Passport and ID of person submitting papers
