Requirements:
Transport of Human Remains
- Certified copy of Death certificate
- Burial/Transit Permit
- Passport, birth certificate or marriage certificate of decedent
- Embalmer's affidavit (notarized with a certificate of notary)
- Letter of non-contagious disease from State or County Health office
- Letter of preparation on funeral home letterhead, notarized and authenticated
- Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
- No requirements on casket
- Zinc lined hermetically sealed wood box
- No consul inspection, legalization fee is $52.00 per each document
- Family must go to the Consulate office with the above documents
- Passport and ID of person submitting papers
Transport of Cremated Remains
- Certified copy of Death certificate
- Cremation Certificate
- Flight Itinerary on funeral home letterhead
- Passport, birth certificate or marriage certificate of decedent
- Family must go to the Consulate office with the above documents
- Passport and ID of person submitting papers
