Requirements:

Transport of Human Remains

1. Two (2) certified copies of the death certificate
2. Iraqi Id for the Deceased
3. Embalming by U.S. standard method
4. Letter of no contagious disease
5. Full name, nationality, place and date of issuance of passport
6. Embalmers affidavit
7. Burial permit/transit permit
8. Use of hermetically sealed casket
9. Use of all wooden outer shipping container
10. Name of person accompanying the remains, with nationality, place of birth, number, date, and place of issuance of passport
