Requirements:
Transport of Human Remains
- Certified copy of Death certificate (Apostille certified from attending medical doctor)
- Post Mortem Certificate
- Burial/Transit Permit
- Passport of decedent
- Embalmer's affidavit (notarized)
- Letter of non-contagious disease (Apostille certified from attending medical doctor)
- Inspection by consulate representative
- Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; Letter must include the name of the deceased, place of birth & death, final disposition and next of kin authorizing arrangements
- Metal casket – hermetically sealed
- All wood shipping container with the bottom of the casket covered with sawdust or similar material which has been moistened with a carbolic acid solution, If the remains are being transported from a warm climate, the body must be treated with formalin or some other substance that resists composition.
Transport of Cremated Remains
- Certified copy of Death certificate (Apostille certified from attending medical doctor)
- Post Mortem Certificate
- Cremation Certificate (notarized)
