Requirements:

Transport of Human Remains

1. Certified copy of Death certificate (Apostille certified from attending medical doctor)
2. Post Mortem Certificate
3. Burial/Transit Permit
4. Passport of decedent
5. Embalmer's affidavit (notarized)
6. Letter of non-contagious disease (Apostille certified from attending medical doctor)
7. Inspection by consulate representative
8. Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; Letter must include the name of the deceased, place of birth & death, final disposition and next of kin authorizing arrangements
9. Metal casket – hermetically sealed
10. All wood shipping container with the bottom of the casket covered with sawdust or similar material which has been moistened with a carbolic acid solution, If the remains are being transported from a warm climate, the body must be treated with formalin or some other substance that resists composition.

Transport of Cremated Remains

1. Certified copy of Death certificate (Apostille certified from attending medical doctor)
2. Post Mortem Certificate
3. Cremation Certificate (notarized)
