Requirements:

Transport of Human Remains

1. Certified copy of Death certificate (2)
2. Burial/Transit Permit (must bear raised seal of the health department)
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease (must bear raised seal of the health department or medical examiner)
6. Funeral home letter stating:
   
   1. That only the remains are inside the casket
   2. Flight itinerary
   3. Consignee's name, address, and telephone number
   4. Cemetery name and location
7. ALL documents must be notarized, authenticated by the county clerk, and then presented to the State Department for Apostille
8. Casket - sealer-type metal
9. Wood shipping box
