Transport of Human Remains

• Certified copy of Death certificate
• Burial/Transit Permit
• Passport or birth certificate of decedent
• Embalmer's affidavit
• Letter of non-contagious disease
• Funeral home letter stating:
  • That only the body is inside the casket;
  • Flight itinerary;
  • Consignee's name, address, and telephone number;
  • Cemetery name and location

• Casket - sealer-type metal
• Air tray (wood bottom)
