1. Certified copies of Death certificate (2)
2. Passport of decedent
3. Burial/Transit Permit
4. Funeral home letter stating:
   
   1. only remains are inside the casket
   2. flight itinerary
   3. consignee's name, address, and telephone number
   4. cemetery name and location
5. Embalmer's affidavit (notarized and authenticated by the County Clerk)
6. Letter of non-contagious disease (notarized and authenticated by the County Clerk)
7. Casket - Metal sealer-type
8. Wood shipping box
