New Legal Form: Receipt of Decedent's Remains by Crematory from Funeral Home

Posted: August 5, 2013

NFDA has added the Receipt of Decedent's Remains by Crematory from Funeral Home form to its library of sample legal forms and documents. The more than 125 legal forms and documents were developed by NFDA General Counsel Scott Gilligan and can easily be downloaded from the NFDA website as a free benefit of membership.

This new form asks a funeral home and crematory representative to acknowledge that the funeral home has transferred posession of a decedent to the crematory and that the crematory has accepted posession of the remains.

You can download this and all sample legal forms and documents from the NFDA website (www.nfda.org/legalforms; login required) at any time and at no cost. You'll find other forms covering situations, such as:

  • Right of disposition
  • Authorization forms (embalming, cremation, disinterment, etc.)
  • Custody and transport of remains
  • Personal property instructions
  • Preneed
  • Funeral Rule
  • Employment forms
  • And more
