One of NFDA's most valuable member resources is its sample forms and legal documents. NFDA has a comprehensive toolbox of nearly 125 forms that were developed by its General Counsel Scott Gilligan. There are forms for virtually every circumstance. The newest is an authorization form for formaldehyde-free embalming.

This new form not only serves as an embalming authorization form, but also includes a clause indicating that the family wants the funeral home to use a formaldehyde-free embalming solution and understands that the results may differ from those of a solution containing formaldehyde.
